Observations
The facility failed to obtain an informed and voluntary consent from the client prior to the disclosure of information in two of ten client records reviewed during the annual licensing inspection on October 18, 2016.
Client #7 was admitted to treatment on 4-13-2016 and discharged on 10-5-2016. Client information was disclosed to a laboratory on April 20, 27, May 4, 18, 25, June 1, 8, 15, 22, 29, July 13, 20, 27, August 3, 10, 22, 31, September 7 and 28, 2016, but there was not a release of information for this laboratory signed by the client.
Client #10 was admitted to treatment on 8-16-2016. Client information was disclosed to a laboratory on October 12, 2016, but there was not a release of information for this laboratory signed by the client.
The findings were reviewed with the facility staff during the licensing process.

Plan of Correction
The Manager of Outpatient Services added a consent into the admission packet for vivitrol patients for the Somerset Hospital laboratory on 11/1/2016. The Manager of Outpatient Services will monitor the vivitrol patients admission packets monthly to ensure that appropriate consents are completed each time. |
