The findings include:
The Methadone Monitoring Questionnaire was reviewed by supervisory staff on May 27, 2011. Per regulation, a drug and alcohol treatment project shall have a full-time clinical supervisor for every eight full-time counselors or counselor assistants, or both. The review of the Methadone Monitoring Questionnaire identified that the facility had 8 full time equivalent counselors in the project, which would require a full time clinical supervisor. There was no documentation that the project employed a full time clinical supervisor.
