This report is a result of an on-site complaint investigation conducted on Febuary 27, 2017 by staff from the Division of Accountability and Program Improvement. Based on the findings of the investigation, Gaudenzia Erie, Inc.- Crossroads was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.

The residential facility shall:
(1) Be free of rodent and insect infestation.

Observations
A physical plant inspection was conducted on Febuary 27, 2017. Based on this inspection, the facility failed to ensure that it was free from insect infestation.

Plan of Correction
Facility Maintenance Supervisor will increase/intensify treatments conducted by the exterminating company from once per month to twice per month starting 3/16/2017.
Program Director will conduct thorough weekly physical plant checks to ensure there is no insect activity starting 3/16/2017.
Program Director will report any activity to the Facility Maintenance Supervisor immediately. |
