INITIAL COMMENTS
 
A follow-up survey conducted on June 10, 2009 found Western Pennsylvania Teen Challenge had corrected the deficiencies cited under chapters of 28 PA Code which pertain to the facility. The deficiencies were cited as a result of an on-site licensure renewal inspection conducted on February 12, 2009.
