INITIAL COMMENTS

This report is a result of an off-site unusual incident report investigation conducted on January 2, 2024 by staff from the Bureau of Program Licensure. Based on the findings of the off-site investigation, Horizon House, Inc. was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
