This report is a result of an on-site licensure renewal inspection conducted on March 19, 2007 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on April 13, 2007.

705.10. Fire safety.
(d) Fire drills. The residential facility shall:
(6) Prepare alternate exit routes to be used during fire drills.

Observations
Exit routes used were not properly documented in the fire drill log. The words "primary" and "secondary" were used to identify the exits used in a building with multiple means of egress.

Safety Officer updated our report of evaluation form to include which exit is being bloced and which exits are being used during the evacuation drill on 03/13/07 |

Observations
The family medical history was missing in 4 out of 5 client records reviewed, specifically #1, 2, 4 and 5.

Family Medical history entry has been added to our Level of Care/Nursing Assestment document in our electronic record (Sigmund). This will be completed upon admission. |
