This report is a result of a complaint investigation conducted July 17, 2025 by staff from the Bureau of Program Licensure. Based on the findings of the complaint investigation Sees the Day, was found to not be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, deficiencies were identified during this investigation.

§ 717.29(b)(1) LICENSURE Physical plant standards - Bldg-Exterior-Grnd
§ 717.29. Physical plant standards.
(b) Building interior, exterior and grounds. The licensee shall:
(1) Keep the interior, exterior, and grounds or yard of the drug and alcohol recovery house clean, safe, sanitary and in good repair and free of hazards at all times.

Observations
Based on the physical plant inspection the recovery house failed to keep the interior, exterior, and grounds or yard of the drug and alcohol recovery house clean, safe, sanitary and in good repair and free of hazards at all times.
Findings: Broken furniture was in the yard and on the balcony of the recovery house. The ceiling leak was next to a electrical light fixture that was uncovered and missing ligh bulbs.

Plan of Correction
Regarding the broken furniture in the your and balcony, all debris and furniture was removed on 07/21/25 by management.
The ceiling leak which was from a window unit air conditioner not properly installed was corrected by house manager the leak was corrected by properly fitting the AC window unit and the uncovered light fixture was properly covered and light bulbs were replaced on 07/17/2025.
Moving forward house managager will be sure to keep house in good repair and free of hazards at all times. |

(3) Keep the drug and alcohol recovery house free of rodent and insect infestation.

Observations
Based on interviews with the residents and the owner the recovery house failed to keep the drug and alcohol recovery house free of rodent and insect infestation.

Plan of Correction
As plan of correction Viking Termite & Pest Control, LLC provided a service on 07/31/2025.
Moving forward we have a follow-up scheduled for 08/12/2025 and as needed. House Management will continue to
monitor this situation keeping the home free of pest. |
