Based on review of the staff records the recovery house failed to provide a personnel file
Application or resume.
(2) A Pennsylvania State Police criminal history record check.
(3) Disciplinary actions.
(4) A written job description for each drug and alcohol recovery house position.
(5) Documentation of training.
These findings were reviewed with the recovery house staff during the licensing process.

Plan of Correction
All staff, employees, or volunteers will complete all DDAP required trainings within 90 days of hire. This will be tracked by the house manager by maintaining a personnel file that includes application/resume, criminal background check, training plan, training certificates, and a brief evaluation of each training. The house manager will be responsible for ensuring that this does not recur. This has been implemented and reinforced on March 10, 2026. |
