Observations
Based on a review of records, the facility failed to obtain an informed and voluntary consent from the client for the disclosure of information contained in the client record in one out of seven records reviewed.

Client #4 was admitted on April 23, 2024 and discharged on May 22, 2024. There was no documentation that the facility obtained an informed and voluntary consent for the funding source. The facility confirmed that the insurance was billed for the services.

This finding was reviewed with facility staff during the licensing process.

Plan of Correction
The Admissions Coordinator or designee is responsible for obtaining the client's initial consent to release information for all external communication, including MCOs. If the funding source changes during the course of the client's treatment stay, the clinician assumes the responsibility in ensuring a release of information is signed for the new funding source. This process will be monitored by the Clinical Supervisor or designee on a biweekly basis. New staff are trained on confidentiality on their first day of onboarding. Treatment Trends Administrative staff will train all new Allentown Residential staff on this process before the program reopens. Once the program is running, the Program Director or Clinical Supervisor will train future staff.
