This report is a result of an a follow-up inspection regarding the plan of correction for the 07/20/2011 licensure renewal inspection. The follow-up inspection was conducted on 01/04/2012 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the follow-up inspection, Mazzitti and Sullivan Counseling Services was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during thie inspection and no plan of correction is required.
