Observations
Based on a review of administrative documentation, the project failed to make available to the public an annual report which includes, but is not limited to: 1) Activities and accomplishments of the preceding year. 2) A financial statement of income and expenses. 3) A statement disclosing the names of officers, directors and principal shareholders.
The findings include:
Documentation was reviewed during the licensing inspection on September 30, 2015. The facility partners confirmed that they did not generate an annual report nor make it available to the public for the year ended 2014 after the Partnership Income Tax Return for 2014 was prepared and received from the Accountant.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
An annual report was prepared and notice of this was made public via a local newspaper on 11/7/15. This will be stored in policy and procedure manual for review as needed.
At he end of the calendar year, The project director (Gina Smithbower-Zabrosky) will publish an annual report of all activities that Impact Counseling Services, LLC have participated in and all accomplishments made by staff at Impact Counseling and as an agency as a whole.
A financial statement with income and expenses will be made public as well as the name of the officers, directors, and principal shareholders of Impact Counseling Services, LLC. |
