This report is a result of an on-site recovery house complaint investigation conducted on July 15, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the on-site complaint investigation, Club Serenity was found not to be in compliance with the applicable chapters of 28 PA Code, Chapter 717, which pertains to the recovery house. Due the provider immediately following policy and procedure, the deficiency is corrected and no POC is required.
