This report is a result of an off-site complaint investigation conducted between Thursday, April 28 2022 and Friday, May 06, 2022 by staff from the Bureau of Program Licensure. Based on the findings of the off-site complaint investigation, Pyramid Helathcare Inc was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this inspection and no plan of correction is required.
