INITIAL COMMENTS

This report is a result of an on-site initial licensure inspection conducted on May 1, 2015 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Visualize Change, LLC, was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiency was identified during this inspection:
As this was the initial inspection conducted, not all regulations were reviewed. During future inspections, all regulations will be reviewed for compliance.

The nonresidential facility shall:
(4) Store all trash, garbage and rubbish in noncombustible, covered containers that prevent the penetration of insects and rodents, and remove it at least once every week.

Observations
Based on observation during the initial licensing inspection of May 1, 2015, it was observed that the facility failed to have noncombustible covered containers that prevent the penetration of insects and rodents.
The findings include:
An initial physical plant inspection was conducted on May 1, 2015 at approximately 11:00 a.m. The building exterior and grounds were observed to have no noncombustible containers that prevent the penetration of insects and rodents.
These findings were reviewed with facility staff during the initial inspection.
