INITIAL COMMENTS
 
This report is a result of offsite unusual incident report investigations conducted during July 3 - 15, 2025. Based on the results of the offsite investigations, French Creek Recovery Center was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
