INITIAL COMMENTS

This report is a result of an on-site complaint investigation conducted on April 9, 2009 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Western Pennsylvania Teen Challenge was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on May 13, 2009. .

Plan of Correction

704.12(a)(3)(i)  LICENSURE NonHosp Rehab

704.12. Full-time equivalent (FTE) maximum client/staff and client/counselor ratios. (a) General requirements. Projects shall be required to comply with the client/staff and client/counselor ratios in paragraphs (1)-(6) during primary care hours. These ratios refer to the total number of clients being treated including clients with diagnoses other than drug and alcohol addiction served in other facets of the project. Family units may be counted as one client. (3) Inpatient nonhospital treatment and rehabilitation (residential treatment and rehabilitation). (i) Projects serving adult clients shall have one FTE counselor for every eight clients.
Observations
Based on an interview with the facility director and a review of the facility staffing summary report, the facility failed to have one Full Time Equivalent (FTE) counselor for every eight clients, as required.

The findings include:

The facility director was interviewed on April 9, 2009. He reports the facility had one counselor leave two weeks ago and the facility is now over the required counselor/client ratio of one FTE counselor for every eight clients. The facility director completed the facility staffing summary report. This confirmed the facility has a current counselor/client ratio of one FTE counselor for every 10 clients; which is over the required 8 to 1 ratio.

Plan of Correction
As of April 23, 2009, we are in full compliance with the counselor to client ratio. On April 23, 2 clients have been discharged and sent to the Teen Challenge Training Center. Now we have one full-time counselor who is carrying an 8 client caseload and a part-time counselor who is carrying a 4 client caseload.

We are currently in the process of hiring a new full time counselor. Until this hiring is official, we will only carry a capacity of 12 clients.
