A physical plant tour was completed on February 18, 2010 at 12:30 PM. At the time of the tour, the door on the Field Street side of the facility was marked as an exit and it also has a key lock. The exit is not accessible without a key. At the time of the tour, this door was locked, and it was confirmed by the outpatient director that due to the key lock, this exit is obstructed.

Plan of Correction
The Clinical Director contacted the landlord upon notice of the deficiency and made arrangements for replacement of the doorknob with one that did not require a key to exit. This was completed March 1, 2010.
