This report is a result of a complaint investigation conducted on January 4, 2024, by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the complaint investigation, the allegations against Pyramid Healthcare, Inc. were unable to be substantiated. Therefore, no deficiencies were identified during this investigation and no plan of correction is required.
