Based on a physical plant inspection, the Project/Facility Director's office included a heater that was not permanently mounted or installed.

The inspection was conducted on February 5, 2015. The Director's office included a space heater that was not permanently mounted or installed.
These finding were reviewed with the facility staff during the licensing process.

Plan of Correction
As of Feb 20, 2015 space heater was removed. If space heaters are used in the future, facility director will contact, head custodian, to permanently mount space heater to wall. Head custodian will affix his signature to documentation concerning mounting space heater. |
