The findings include:

The facility's census on September 2, 2010 consisted of 52 clients from the Drug and Alcohol Program.

A tour of the facility ' s physical plant was conducted on September 2, 2010 at approximately 12:30 AM. The facility was required to be free of rodent and insect infestation. At the time of inspection, the facility had a bedbug infestation.

At the time of inspection, 13 clients from the Drug and Alcohol (D&A) program and 28 clients from the Mental Health (MH) program were sleeping on camping-style folding cots in the gymnasium. The remaining 39 D&A clients were still residing in the building that had the bed bug infestation.

An interview with the program manager was conducted on September 2, 2010 at approximately 1:30 AM. The program manager confirmed that the facility currently had a bedbug infestation. The program manager confirmed that the 13 clients from the D&A program and 28 clients from the MH program were sleeping in the gymnasium and that the remaining D&A clients were in the building that had the bed bug infestation.

Plan of Correction
1. Once an incident of bed bugs has been confirmed that a dorm/room has Bed Bugs, the Maintenance staff and Program Manager will inspect the infected area, to determine the plan of action.

2. All clothing and bedding will then be place in clear plastic bags, to ensure no bugs will escape. The laundry will then be passed through a commercial dryer, to apply heat of 130 degrees, to kill any bugs on the clothing/bedding.

3. The maintenance dept. will then determine if the amount of bugs will need insecticide, or heat treatment. (Depending on amount of bugs found in room, it will determine what treatment is used.

4. Insecticide Treatment- Will consist of all belongings being removed from the infected area. A licensed exterminator will then spray infected area with insecticide. Once the insecticide dries, the room will be re-inspected, to make sure Bed Bugs are dead. Once final inspection has been made by Program Manager, the custodial dept. will then clean the room. In addition, once all aforesaid measures have been taken, the students will then be allowed to move back into the rooms.

5. Heat Treatment- Once report of Bed Bugs has been confirmed, Maintenance Staff and Program Manager will inspect the infected room to determine the plan of action.

6. Once it is determined that infestation warrants the Heat Treatment, all clothing and bedding will be placed in clear plastic bags. The laundry will then be taken to commercial dryer to be heated to 130 degrees to insure the Bed Bugs are killed.

7. Once all belongings are removed from the room/dorm, all wooden furniture i.e. bed frames, drawers, etc... Once all wooden furniture is disassembled and the rooms will be heated to a minimum temperature of 150 degrees for two hours.

8. Once Heat Treatment is over, furniture will be resembled. All cracks and crevasses will be caulked and sealed and furniture panted with an epoxy paint returned to rooms. A final insecticide spray will be made. Custodial Dept. will then clean the infected rooms.

9. Once rooms are cleaned and re-inspected by Maintenance Staff, the Program Manager will make a final inspection to ensure that bugs have been killed. If room/dorm is cleared, the students may return to room/dorm.

10. A maintenance spray will be used weekly, to further ensure that the Bed Bugs do not return. Steri-Fab is the spray that will be used on bed frames, and wooden drawers. No contact with any students' clothes or persons will be permitted.

Teen Challenge has adjusted protocol to treat further possible infestations to help assure that the Drug and Alcohol students are placed in rooms with no more than four men. By setting aside an allotment of two (four man) rooms (that meet the state's standards for Drug and Alcohol programs) for alternate housing options in case of further infestations
