Observations
Based on an onsite investigation which resulted in staff and client interviews along with a review of client records, the facility failed to provide individual counseling, at least twice weekly to clients enrolled in its PHP program.
These findings were reviewed with facility staff during the compliant investigation process.

Plan of Correction
Effective 4/3/23, clients enrolled in PHP level of care will receive 2 individual sessions per week. The PHP counselor or other identified counselors will be assigned times before, after and during lunch break to meet with clients. These sessions will be documented in each clients charts. On Friday 3/31/23, staff will be educated on this new protocol as it will be reviewed with them in meeting with supervisor. Program Coordinator will be responsible for oversight of ensuring this POC is being met. Program Coordinator will monitor this through chart audits and during supervision with staff. |
