INITIAL COMMENTS

This report is a result of an off-site complaint investigation conducted on July15, 2025 through July 18, 2025 by staff from the Bureau of Program Licensure. Based on the findings of the off-site complaint investigation, DQRT6701 Unity Sober Living was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the recovery house. Therefore, no plan of correction is required.
