Observations A physical plant inspection was conducted on January 4, 2017. Based on this inspection, the facility failed to keep the grounds of the facility clean, safe, sanitary and in good repair at all times for the safety and well being of clients, employees and visitors. There were unsecured electrical wires in the area leading to the rear fire exit. There was a large hole through the wall to the exterior that would allow for rodents and/or insects to enter the building. The exit discharge for the intended second exit discharges directly to an alley way where cars do come in and out of the area adjacent to the building, thus, presenting a hazard of getting hit by a car. The exterior of the building had and extensive amount of poison ivy around the door and up the building. These findings were reviewed with facility staff at the time of the inspection.
