Plan of Correction 709.92(b) Treatment and Rehabilitation Services
1). An immediate plan of correction took place on Monday, August 28, 2023 updating the current treatment and rehabilitation plans to be reviewed and updated at least every 60 days that will include the following:
709.92 (b)
Treatment and rehabilitation plans shall be reviewed and updated at least every 60 days by the counselor and client. The following areas should be considered as an assessment of the client's progress in relationship to the stated goals of the comprehensive treatment plan:
*Have problems, issues, or goals that have been identified in the treatment plan been impacted upon through treatment?
*Do goals need revised or restated?
*Do treatment strategies or action steps need to be modified?
*Is closure reflective when goals are achieved?
The Treatment Plan is to be signed and dated by the counselor and client. However, in the event the client services have a pause, not a discharge, in services and the client is unavailable for accurate update,
ie: client #2 and #7, the counselor will restate the previous treatment plan goals and action steps by the 60 day mark and note in the progress section that an accurate update will be conducted at the next scheduled session with the client.
2).The Project Facility Director will be responsible for achieving and maintaining compliance standards on a monthly basis.
3). The Turning Point II, Inc. has completed the Plan of Correction and updated the policy and procedure binder.
