INITIAL COMMENTS

This report is a result of an on-site inspection conducted for the approval to use a narcotic agent, specifically methadone, in the treatment of narcotic addiction. This inspection was conducted on June 30, 2008 through July 2, 2008 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Parkside Recovery was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Deficiencies were identified during this inspection and plan of correction is due on July 29, 2008.

Plan of Correction

715.7(b)  LICENSURE Dispensing or Administering Staffing

(b) Dispensing time shall be prorated for patient census. There shall be sufficient dispensing staff to ensure that all patients are medicated within 15 minutes of arrival at the dispensing area.
Observations
Based on observation during the three day monitoring inspection, the facility failed to ensure that patients are medicated within 15 minutes. The process observed had all patients that had been in group counseling waiting to be medicated with any other patients needing medication being inserted into the patients that were already waiting. The time observed was almost 60 minutes to medicate a patient and complete the process.

Plan of Correction
The Program Director will Provide oversight with compliance to regulation 715.7(b) by working with the Clinical Supervisor to develop and institute a walk-in medication schedule for patients not scheduled for medication after the group process.

The Program Director will review the regulation pertaining to the deficiency during staff training on 8/4/08. The Program Director will instruct The Clinical, Medical, and Security staff to stop inserting walk-in patients into the group medication process between the hours of 11:00 am to 1:00 pm Monday to Wednesday, and 4:30pm to 6:30pm Tuesday to Thursday.

All patients attending the program will receive a copy of the walk-in medication schedule by their primary Counselor in their group/individual sessions during the week of 8/4/08. The patient will be asked to sign an acknowledgment form documenting the notification of the walk-in medication schedule. The schedule will identify times when walk-in patients may come into the clinic to be medicated that will not interfere with the group medication process.

The program will be in full compliance with standard 715.7(b) on 8/18/08.
