Plan of Correction
|
Observations Based on a review of client records, the facility failed to inform the Department of an unusual incident within the required three days. It was discovered that the facility had an unusual incident that occurred December 9, 2025, involving two clients physically assaulting members of the treatment community. Upon further inspection, it was discovered that the facility had not submitted a written unusual incident report to the Department within the regulatory three business day timeframe.
This finding was reviewed with project and facility staff during the complaint investigation process.

Plan of Correction The failure to submit an unusual incident report to the Department came from an oversight on the part of the Program Director, Joe Friel.
In responding to the seriousness of this incident, Mr. Friel incorrectly assumed that the Childline Referral made by our program staff would satisfy the reporting requirements to the Department.
This has been corrected through communication to all departments, the requirements regarding submitting unusual incident reports to DDAP. All incidents involving submitting a Childline report will be followed by a meeting of all department supervisors to ensure that each aspect of communication required has been completed.
This obligation will be supervised by the Executive Director, but will remain the responsibility of the Program Director. |
