Based on the review of the facility's fire drill log for the months of August 2024 through July 2025, the facility failed to prepare alternate exits to be used during monthly fire drills.
These findings were reviewed with facility staff during the licensing process.

705.28 (d) Fire Safety
To ensure that Axiom Family Counseling Inc., follows fire safety regulations at the New Kensington facility, the Facility Director reviewed safety regulations on 8/6/2025 and will conduct monthly fire drills using alternative exits at least twice per year. The fire drill log will be audited by the Facility Director each month to ensure compliance with DDAP fire safety regulations.
