This report is a result of an on-site licensure renewal inspection conducted on April 5, 2023, by staff from the Department of Drug and Alcohol Programs, Bureau of Program Licensure. Based on the findings of the on-site inspection, Child Guidance Resource Centers was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:

§ 709.22. Governing body.
(c) If a facility is publicly funded, the governing body shall make available to the public an annual report which includes, but is not limited to, a statement disclosing the names of officers, directors and principal shareholders, when applicable.

Observations
Based on a review of administrative documents and discussion with facility staff, the facility failed to make available to the public an annual report that included a statement disclosing the names of officers, directors and principal shareholders.
The last annual report documented on the facility ' s website was the 2020 annual report.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
The annual report is currently being worked on. The Project Director addressed this plan of correction with the CEO at CGRC, and was informed that it will be completed and posted on the website by the end of August 2023. At that time, Project Director can forward a copy with the web address to this department if needed. The CEO will ensure that the annual report is completed and available on the website for the public to view. Moving forward, the CEO will ensure that the annual report is completed on a regular basis and placed on our website. This will be monitored by the board of directors at CGRC during a board meeting. |
