Observations
Based on a December 17 through 18, 2007 review of client records, the facility failed to document the duration of group therapy sessions in four of four client records reviewed, # 1, 2, 3 and 4.

Plan of Correction
Program Director directed Administrative asistant on December 19, 2007 to modify record of service to include duration of group therpay sessions. Document was implemented on 12/31/2007. Counselors will be responsible for completing record of service for group therapy by initialing as well as marking the duration for each group session. Group therapy lasts approximately one and one half hours.
Program Director will monitor charting procedures monthly to ensure records of service are being recorded appropriately. |
