Observations The project failed to secure and maintain client records in a locked storage container.
On July 2, 2020 the Department was contacted by a self-storage facility that had discovered client records from this project in one of the storage lockers at their site. It became necessary for the storage facility to open the locker after attempts to contact the provider to obtain overdue payment on the storage unit were unsuccessful.
A conversation with the facility staff revealed that these records were from discharged clients from one the facilities in the project that was no longer operating, therefore this deficiency is being cited under the project headquarters.

Plan of Correction The project failed to secure and maintain client records in a locked storage container.
The project headquarters was notified regarding the failure to maintain compliance with payment for the storage facility to continue to provide appropriate storage for corporate records and client records from now closed facilities in our system. The storage facility was immediately contacted and payment was made in full to continue to maintain records in this secured location. The facility director also made arrangements with the storage facility to ensure that this will not occur again by setting up an automatic monthly payment for the unit. The facility director has contacted the company attorney and will follow all directions from the attorney in regard to any additional reporting that needs to be implemented. The Facility is compliant with maintaining records in a secure locked storage container as of July 3, 2020 |
