INITIAL COMMENTS

This report is a result of an on-site licensure renewal inspection conducted on August 7, 2008 through August 8, 2008 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Catholic Charities Diocese of Harrisburg PA Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on September 5, 2008.

Plan of Correction

709.32(c)(4)  LICENSURE Medication Control

709.32. Medication control. (c) The project shall have a written policy regarding medications used by clients which shall include, but not be limited to: (4) Methods for control and accountability of drugs.
Observations
Based on a review of the medication policies and procedures and medication administration records, the facility failed to document complete medication records.

Finding:

The medication records were blank on numerous days in June 2008 and July 2008. Documentation was missing to indicate whether or not a client received his/her medication, refused his/her medication or whether the medication was discontinued.

Plan of Correction
1. The Program Director will hire, assign, and train a newly hired part time employee to complete a medication audit every other week which would include looking for errors and incomplete documentation. This would be her sole job every other week. (Hired 8/14/08 and med training started 8/28/08)

2. The Program Director will continue to complete a staff-wide training on medications and documentation each year. (11/08)

3. The Program Director and the Associate Executive Director will enforce stiffer penalties for medication errors based on biweekly reports generated from the newly trained and hired employee cross-checking the medication logs, etc. Penalties would include Write-Ups for every few mistakes and eventually recommendation for termination if consistent errors occurs for a particular staff person even after several warnings and memos. (Ongoing)
