This report is a result of an on-site licensure renewal inspection conducted on June 14, 2006 by staff from the Division of Drug and Alcohol Program Licensure. The following deficiencies were identified during this inspection and a plan of correction is due on July 14, 2006.

709.93. Client records.
(a) There shall be a complete client record on an individual which includes information relative to the client's involvement with the project. This shall include, but not be limited to, the following:

Observations
Aftercare plans did not include goals with time frames in five of five charts reviewed.

Plan of Correction
By July 17, 2006 the Commission's "Aftercare Plan" template will be modified to clearly prompt the D&A Treatment Specialist & his/her Patient to enter aftercare plan goals, appropriate time frames to achieve the goals, as well as the barriers they may encounter & supports they might employ when working on the goals of their aftercare plan. The new "Aftercare Plan" template and accompanying policy & procedure memo will be forwarded to all of the Commission's outpatient treatment facilities (Lehighton: Facility #131044, Stroudsburg: Facility #451044 and Honesdale: Facility #647012). |
