This report is a result of an renewal on-site licensure inspection conducted on October 7, 2025 by staff from the Bureau of Program Licensure. Based on the findings of the on-site inspection, THDF6701 Unity Sober Living Homes was found to not be in compliance with the applicable chapters of 28 PA Code, Chapter 717 which pertains to Recovery House licensing.

§ 717.20. Resident records.
(a) The licensee shall maintain an individual record for each resident which must include:
(1) All records obtained under § 717.22.

The recovery house failed to maintain an individual record for each resident.

Plan of Correction
We have set new organization requirements so this does not happen again. The director of operations will implement the new system by 10/15/25 and do continuous audits of all paperwork quarterly to ensure this doesnt happen again. |
