INITIAL COMMENTS
 
This report is a result of an unusual incident investigation conducted off-site on October, 23, 2025, and onsite on November 6, 2025, by staff from the Bureau of Program Licensure. Based on the findings of the unusual incident investigation, Cove Forge Behavioral Health System at Williamsburg was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility.
