This report is a result of onsite incident report investigation conducted on October 5, 2015, by staff from the Program Licensure Division. Based on the findings of the investigation, the allegations made against Community Guidance Center were substantiated. Community Guidance Center was found to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. Therefore, no deficiencies were identified during this inspection and no plan of correction is required.
