This report is a result of an on-site complaint investigation conducted on 12/17/2009 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site complaint investigation at Discovery House Cu - Inc., the complaint allegations were not substantiated. No further action is required by the facility.
