Observations
Based on a review of staff records, the facility failed to ensure that each counselor completed at least 25 clock hours of training annually in one of one applicable records. Staff # 3 was hired to the position of counselor on November 30, 2015 and was in this position at the time of review. Staff #3 did not complete at least 25 hours of training throughout the January 1, 2025 through December 31, 2025 training year. These findings were reviewed with facility staff during the licensing process.This is a repeat citation from the September 25, 2025 licensing inspection.

Plan of Correction
Management and the staff member that did not have his training hours completed had a long discussion and came up with a training hour plan. This plan was to be executed beginning 2/16/2026.
All staff will be required to attend an in house staff meeting, in which we review where to obtain them from the internet. Also discussed with managers that they need to teach their direct staff hours, complete an audit of what they have submitted to HR. All meetings and audits will be ongoing. The management team will begin to plan on 3/1/2026
