INITIAL COMMENTS

This report is a result of a supervisory review of the written report of the findings of an on-site licensure inspection that was conducted on March 1, 2017, by staff from the Division of Drug and Alcohol Program Licensure. Based on the supervisory review of findings from the on-site inspection, Turning Point Alternative Living Solutions, was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this supervisory review:

Plan of Correction

704.9(a)  LICENSURE Counselor Asst Supervision

704.9. Supervision of counselor assistant. (a) Supervision. A counselor assistant shall be supervised by a full-time clinical supervisor or counselor who meets the qualifications in 704.6 or 704.7 (relating to qualifications for the position of clinical supervisor; and qualifications for the position of counselor).
Observations
Based upon a review of the Staffing Requirements Facility Summary Report (SRFSR)completed by the facility on March 1, 2017, the facility failed to ensure that counselor assistants were supervised by a full-time clinical supervisor.

The SRFSR completed on March 1, 2017 listed the clinical supervisor as carrying a caseload of 8 clients and dedicating 30 hours of the 40 hour workweek to the provision of services for those clients. Based on the number of clients reported on the clinical supervisor's caseload and the number of hours provided to client care, the clinical supervisor was not a full time clinical supervisor.

Plan of Correction
The clinical supervisor position is a full time position, when the staffing grid was completed we were under the assumption that hours spent supervising would be listed as hours with drug and alcohol clients because of supervising the counselor assistants while seeing their clients. Next year when the staffing grid is completed, we will no longer list the hours this way. The clinical supervisor has a small case load to allow for time for supervision, and the majority of her time is spent supervising. She will maintain a caseload of no more than 5 clients to also maintain her counseling skills, as of our last census meeting in 4/26/2017 the Facility Director ensured the clinical supervisor had only 5 clients on her caseload, this will be continuously monitored by the Facility Director. In reality if the hours spent supervising are supposed to be listed as other then the correct break down would be 10 hours spent with clients and 30 hours as other.

704.11(c)(1)  LICENSURE Mandatory Communicable Disease Training

704.11. Staff development program. (c) General training requirements. (1) Staff persons and volunteers shall receive a minimum of 6 hours of HIV/AIDS and at least 4 hours of tuberculosis, sexually transmitted diseases and other health related topics training using a Department approved curriculum. Counselors and counselor assistants shall complete the training within the first year of employment. All other staff shall complete the training within the first 2 years of employment.
Observations
Based upon a review of the Staffing Requirements Facility Summary Report (SRFSR)completed by the facility on March 1, 2017, the facility failed to ensure that staff persons received a minimum of 6 hours of HIV/AIDS and at least 4 hours of tuberculosis, sexually transmitted diseases and other health related topics training using a Department approved curriculum within the required timeframe.

The SRFSR completed on March 1, 2017 listed the project/Facility Director as only having completed one hour of the required four hours of TB/STD and other health related topics training. The SRFSR listed the Project/Facility Director as having completed seven hours of HIV/AIDS training on 10/13/2014; however, the facility failed to list the name of the provider for the HIV/AIDS training to verify that the training was in accordance wirth the Department approved curriculum.

Plan of Correction
The staffing grid was completed incorrectly, the number of hours spent at the TB/STD training was 6 and it was a DDAP training, the HIV training was the 7 hour training and the name of the provider is listed. The name of the provider is CE Units and they are listed as being an approved trainer by the American Psychological Association.

We were not aware that this was not an approved training and the Facility director will schedule a DDAP approved training as soon as one becomes available. Once this training is received it, and the other training will be filled n correctly on the staffing grid for our next inspection.
