INITIAL COMMENTS
This report is a result of an on-site licensure renewal inspection conducted on 01/19/2016, and 01/20/2016, by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Crossroads Counseling Inc., was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection:	Plan of Correction
709.84(a) LICENSURE Project management services 709.84. Project management services. (a) The partial hospitalization project shall operate at least 5 days of the week and for a minimum of 40 hours per week. Additional hours should be appropriate to the population served by the partial hospitalization project.
Observations Based on staff interviews conducted during the onsite inspection for 01/19/2016, and 01/20/2016, and a review of the facilities official website the facility does not provide partial hospitalization hours for a minimum of 40 hours per week. The findings include: The official website for the facility reads, "Levels of Care: Outpatient, Intensive Outpatient, Partial Hospitalization Services (Partial Program operates Mon-Fri 8:30-12:30)" The hours posted at the facility also document that the Partial Hospitalization Program only operates Monday through Friday from 8:30AM to 12:30PM.	Plan of Correction Our office is open Monday through Thursday from 8am to 9pm as well as Fridays and Saturdays from 8am to 2pm. Partial hospitalization services are available during these hours. The information indicating that services are limited to 8:30-12:30 has been removed and correct information is displayed. The Facility Director will be responsible to ensure that accurate hours are always posted both at the physical site and on our official webpage.
