Observations
Based on a review of client records, the facility failed to document a buprenorphine consent to treatment in client record #3.
Client #3 was admitted to treatment on March 2, 2020 and is currently an active client. Client #3 received the first buprenorphine prescription on May 29, 2020. There was no buprenorphine consent to treatment in client record #3.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
Buprenorphine consent will be obtained for all buprenorphine clients prior to receiving their first buprenorphine prescription. This consent will be collected and maintained in the Electronic Health Record.
As follow-up procedure, the Program Supervisor will conduct chart reviews of all buprenorphine clients for three months to ensure buprenorphine consent has been obtained by all buprenorphine clients. The corrective plan of action will be in effect 3/15/21. |
