INITIAL COMMENTS
This report is a result of an on-site licensure renewal inspection conducted on February 11-13, 2008 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Renewal Treatment, Inc was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on March 18, 2008.	Plan of Correction
709.28(c) LICENSURE Confidentiality 709.28. Confidentiality. (c) The project shall obtain an informed and voluntary consent from the client for the disclosure of information contained in the client record. The consent shall be in writing and include, but not be limited to:
Observations Based on the review of client records, the facility failed to obtain an informed and voluntary consent for the disclosure of client identifying information in nine of seventeen records reviewed, #1, 2, 3, 4, 5, 11, 12, 16 and 17. Consent to release information forms signed by the client stated that information released to a non health care provider or an insurance agency could be re released without the client's consent. This is a violation of federal regulations at 42 CFR Part 2, Subpart C, Subsection 2.32. In two of seventeen records, #5 and 16, a blank consent to release information forms was signed by the client and the staff.	Plan of Correction Renewal Treatment Inc. shall recieve informed voluntary consent for the disclosure of client identifying information. Renewal Treatment Inc. shall obtain Consent to Release documentation signed by the client that is in compliance with federal regulations at 42 CFR Part 2, Subpart C, Subsection 2.32. Renewal Treatment Inc. shall change the Consent to Release Information form to comply with the Federal regulation. Bill Bernardo, Project Director, Carol Lee Kelly, Clinical Supervisor and Rae Ann Gordon, Clinical Supervisor shall change the form to ensure compliance, provide staff with training and provide supervisory follow-up to ensure compliance. This shall be completed by March 20, 2008.
