Observations
Based on applicable records reviewed, the facility failed to provide an individual treatment and rehabilitation plan developed with the client to include the type and frequency of treatment and rehabilitation services in two of three records.
Client #2 was admitted on January 24, 2024 and was discharged on September 25, 2024. A comprehensive treatment and rehabilitation plan was developed with the client on February 28, 2024 however, there was no documentation of type and frequency of services.
Client #3 was admitted on December 20, 2023 and was still active at the time of the inspection. A comprehensive treatment and rehabilitation plan was developed with the client on January 5, 2024 however, there was no documentation of type and frequency of services.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
This was discussed in meeting with Clinical Supervisor and Counselors. All new counselors will be trained and Clinical Supervisor and Director will review tx plan monthly and ensure that those 2 areas are in each tx plan objective.
This will be reviewed again in next Clinical Staff Meeting on 12/5/24 |
Observations
Based on a review of client records, the facility failed to review and update treatment and rehabilitation plans at least every sixty days in six of seven applicable records reviewed.
Client #1 was admitted on September 30, 2020 and was still active at the time of the inspection. The facility had a Department-approved exception allowing this client ' s treatment plan updates to be completed every 120 days. A treatment plan update was completed on March 29, 2024 and the next update was due no later than July 29, 2024; however, the update was completed on August 1, 2024.
Client #2 was admitted on January 24, 2024 and was discharged on September 25, 2024. A treatment plan update was completed on April 4, 2024 and the next update was due no later than June 4, 2024; however, the update was completed on July 31, 2024.
Client #4 was admitted on March 18, 2024 and was still active at the time of the inspection. A treatment plan update was completed on May 16, 2024 and the next update was due no later than July 16, 2024; however, the update was completed on July 31, 2024.
Client #5 was admitted on March 27, 2023 and was discharged on June 25, 2024. The facility had a Department-approved exception allowing this client ' s treatment plan updates to be completed every 120 days. A treatment plan update was completed on January 11, 2024 and the next update was due no later than May 11, 2024; however, the update was completed on May 20, 2024.
Client #6 was admitted on January 10, 1995 and was still active at the time of the inspection. The facility had a Department-approved exception allowing this client ' s treatment plan updates to be completed every 120 days. A treatment plan update was completed on January 10, 2024 and the next update was due no later than May 10, 2024; however, the update was completed on May 22, 2024. Additionally, the next update was due no later than September 22, 2024; however, the update was completed on September 30, 2024.
Client #7 was admitted on September 23, 2020 and was discharged on February 29, 2024. A treatment plan update was completed on November 7, 2023 and the next update was due no later than January 7, 2024; however, the update was completed on January 11, 2024.
These findings were reviewed with facility staff during the licensing process.

Plan of Correction
Pittsburgh CTC will work with training on new staff and current staff on tx plan updates.
We will go over a training in next Clinical Staff Meeting on 12/12/24 on updating tx plans. The Clinical Supervisor and Director will also check with IT to see if there is a way to change the dates on the tx plan. Clinical Supervisor will encourage cs to work close to the due date for each tx plan. Clinical Supervisor and Director will review tx plans monthly to montior for compliance.
