Plan of Correction The Facility Director will conduct a staff training on 2-25-15 on the discharge process for clients in treatment. Counselors will be educated about the proper notification to clients when there is an involuntary termination. Facility Director will also educate counselors how to clearly document on letter of discharge that "Client is being discharged" and also clearly state the reasons for termination. Facility Director will randomly audit charts on a monthly basis to ensure that the standard is met. Corrective action will be completed by 5-25-2015. |
