INITIAL COMMENTS

This report is a result of an on-site inspection conducted for the approval to use a narcotic agent, specifically methadone, in the treatment of narcotic addiction. This inspection was conducted on December 21-23, 2009 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Alliance Medical Services was found not to be in compliance with the applicable chapters of 4 PA Code and 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on January 16, 2010.

Plan of Correction

715.6(d)  LICENSURE Physician Staffing

(d) A narcotic treatment program shall provide narcotic treatment physician services at least 1 hour per week onsite for every ten patients
Observations
Based on the review of time sheets, census reports and an interview with the facility director, the facility failed to provide at least one hour of physician time per week, on site, for every ten patients.

The findings include:

On December 21-23, 2009, census reports and physician time sheets were reviewed for the months of September, October, November and December 2009. There were insufficient onsite physician hours in September 2009.

During the week of September 7-11, 2009, the census was 398 patients but only 36 physician hours were documented.

The facility director was interviewed at approximately 9:00 AM on December 22, 2009. The facility director confirmed there were insufficient physician hours from September 7-11, 2009.

Plan of Correction
Facility Director will review census at the end of each week and schedule the physician and physician's assistant hours to surpass the 1:10 hour to patient ratio. The physician's hours and the back up physician will be scheduled for extra hours if the physician's asst. is scheduled off for any hours. The back up physician will be contacted to cover any hours that the physician can not cover due to vacation, conference, other work responsibilities or emergencies.
