Observations
Based on a review of staff records, the facility failed to provide documentation of at least 25 clock hours of annual training required for a counselor for the training year July 2022-June 2023 in one of one applicable records.

Staff #3 was hired as a counselor on June 7, 2007 and was still in this position at the time of the inspection. During the review of the staff record, 3.5 training hours were able to be verified.

These findings were reviewed with facility staff during the licensing process.

Plan of Correction
The facility director will re-train all team members on the training requirements for their position, including the number of hours needed per training year, what training topics would be applicable, and the various training resources within POWER and outside of the agency. The internal process for turning in training certificates and feedback forms will also be reviewed. This will be completed at the next team meeting on 1/4/2024.

The facility director has developed a training tracker to enter training hours and dates as they are completed by team members. The facility director will review it quarterly to monitor and ensure training hours are being completed in a timely manner and it will be reviewed with each team member in supervision.
