Observations
During a supervisory review of your licensing packet, the reviewer determined the facility failed to ensure that staff caseloads not exceed 35 active clients.

The findings include:

A supervisory review was completed for the written report of the findings of the on-site licensing inspection that was conducted on November 7-8, 2011. Based on a review of the Staffing Requirements Facility Summary Report, completed by the facility, the supervisory review determined that the facility failed to ensure that the caseload for personnel # 3 not exceed 35 active clients. Personnel # 3's caseload equaled 60 active clients.

The actual client caseload is determined by dividing the Full Time Equivalent (FTE) into the actual number of active clients on a caseload. The FTE is determined by dividing the number of hours devoted to the client's treatment by the facility ' s standard workweek.

The number of hours per week devoted by Employee # 3 to client treatment, as reported by the facility on the Staffing Requirements Facility Summary Report, was 10 hours per week. The facility reported on the Staffing Requirements Facility Summary Report form that Employee # 3 had 15 active clients on November 8, 2011.

Employee # 3 (10/40 = .25 FTE 15 clients/.25 FTE = 60:1 caseload)

Plan of Correction
Employee #3 who has 10 hours clinical time has seven clients on her caseload. Adolescent supervisor has 25 hours clinical time and has eight clients on her caseload. The Adolescent supervisor will go over employee #3 caseload daily to ensure this deficiency does not re-occur.
