Observations
Based on a review of the facility's fire drill logs from April 2021 through January 2022, the facility failed to ensure that fire drills were conducted on different days of the week. The facility conducted all fire drills on Thursdays.
These finding were reviewed with facility staff during the licensing process.

Plan of Correction
Crossroads Counseling, Inc. fully intends to comply with all requirements regarding fire safety and fire drills.
Due to Covid, we had limited staffing in the building the past year. We thought it best to hold our fire drills monthly during our staff meeting as this was when we had the most people in the building. Effective 3/14/2022, all the office managers have been reminded that fire drills must be conducted on differing days of the week and the schedule has been updated to make sure they do not always occur on staff meeting day. The Director will follow up quarterly to ensure continued compliance.
