INITIAL COMMENTS
This report is a result of an on-site licensure renewal inspection conducted on March 4-7, 2008 by staff from the Division of Drug and Alcohol Program Licensure. Based on the findings of the on-site inspection, Crossroads Counseling , Inc. was found not to be in compliance with the applicable chapters of 28 PA Code which pertain to the facility. The following deficiencies were identified during this inspection and a plan of correction is due on April 3, 2008.	Plan of Correction
709.92(a) LICENSURE Treatment and rehabilitation services 709.92. Treatment and rehabilitation services. (a) An individual treatment and rehabilitation plan shall be developed with a client. This plan shall include, but not be limited to, written documentation of:
Observations Based on a review of client records during the onsite licensing inspection held March 4-7, 2008, the facility failed to consistently follow the time frames established in the policy and procedure manual in formulating individual treatment plans in five of fourteen records reviewed. Refer to client records # 1, 4, 6, 10 and 12.	Plan of Correction Crossroads Counseling, Inc. intends to fully comply with all treatment planning requirements. We recognize that treatment plans need completed according to timeframes established in the policy and procedure manual. Staff will be retrained on treatment planning requirements as well as the required timeframes on 3/25/2008. The Executive Director, William Knecht, will provide this training. In addition, Clinical supervisors will review and sign off on all treatment plans to assure that they are being completed accurately and timely.
