Based on a review of six client records on September 3, 2008, the facility failed to obtain an informed and voluntary consent from the client for the disclosure of information contained in the client record in six of six client records reviewed.

Findings:

The consent to release information forms contained language such as "release to" and/or "obtain from" which does not provide for an informed consent. Facility Director will develop a new release form for "release to" only. The Facility Director will re-write the release form to include the statement that clients can revoke a release either verbally or in written, dated communication to the therapist or the CORE Program Director. The Facility Director will inform staff of changes 10/1/08.
