This report is a result of an off site phone contact with the provider following an intervention request and complaint from a patient. A phone interview was conducted with facility staff on July 13, 2011 by staff from the Division of Drug and Alcohol Program Licensure. Based on the information obtained from the patient and the responses from facility staff, there were no violations of regulations and no deficiencies were identified. A plan of correction is not required.
